Park Place was founded in 1962 and offers 165 beds. Residents recuperating from surgery or illness can regain their independence and functionality at Park Place Nursing and Rehab. Every patient’s singular needs are addressed in a well-coordinated personal plan of care.
 
Patient care is delivered by an interdisciplinary team that includes registered nurses, therapists, and certified professionals who use progressive technology and strong fundamental rehabilitation techniques. Our highly qualified staff deliver a treatment plan in a rehabilitative environment customized for each individual, with treatments determined by the patient, clinical staff, and attending physician.
 
Integrated clinical programs include proven medical technology, evidence-based treatment protocols, advanced therapist training, and ongoing support to improve clinical outcomes.
 
Park Place Nursing and Rehab offers a full range of services including 24-hour, full-service sub-acute rehabilitation, skilled nursing, and long-term care, as well as specialized diabetes, cardiac, orthopedic, pulmonary and neurological rehabilitation programs. We strive to create a warm and welcoming environment with five-star amenities that help people heal faster and live more fully.

Park Place is a wonderful facility! It is always clean a pretty inside, and they take care of their residence very well. My mother-in-law is there and has no issues to complain about. They have an excellent activities program to keep the residences active. We are glad we chose Park Place.

My Family's Positive Experience, Part 3 (Final)I’ll close by praising the therapy team at Park Place. They are a bunch of Rock Stars! As noted above, what mattered most to my family was mom being in a facility with a robust therapy component. Park Place shines in this area! Mom received PT, OT, and Speech; she was on the schedule at least 5x per week. Jasmine leads the team and she is a master scheduler. I don’t know how she does it, but all residents receive a healthy dosage of services. We were thoroughly impressed with all the therapists and the folks that stick out the most are Will (PT), Leah (ST), and Kaitlyn (OT). You can’t miss Will. Unless I’m there, he’ll likely be the tallest person in the building. Will’s style was just what mom needed. He was assertive enough to push mom past her comfort zone in their sessions without wearing her out. He not only worked with mom but also educated and coached our family on how we can best support mom’s ongoing recovery. I am especially grateful for the transfer training sessions we had with Will. Because of the time he invested, we are capable of safely assisting mom in transitioning from wheelchair to car which is very useful in taking your loved one to OP appointments. Will truly has a servant’s heart and care’s for the individuals he serves. Leah was amazing! Having little familiarity with Speech Therapy, I can only imagine that Leah is the example that most speech therapists aspire to be. The passion for her work and love of people is evident in her interactions. Lastly, if my wife were writing this review, she would be raving about Kaitlyn. My kudos and testament to the impact Kaitlyn made is evident in the increased confidence my wife has for assisting mom in all things related to grooming and dressing.I realize this 3 Part review is a long post, but it had to be because so many people at Park Place showed my mother and family love and support during a very trying and hard time. I'll remain ever so grateful for the care we received and though I hope to never need the services again, the only place I’ll consider for SNF care for a loved one in the future will be Park Place.

My Family's Positive Experience, Part 2The business office and social work teams deserve a nod too. Megan in the business office was very helpful. I really appreciate that it never felt like finances was what mattered at Park Place. It always felt like mom’s care was front and center. As far as case management and care coordination activities are concerned, Anna was awesome. Mom has a Medicare Advantage Plan and we quickly learned that UHC can be a little stingy with days covered. We received the first Notice of Non-Coverage from UHC before mom was strong enough to safely discharge home. Anna stewarded me through the appeals process every step of the way. She was an ally and an advocate. The first appeal was denied but we persisted and appealed a 2nd time. We provided more information and fortunately the 2nd appeal was approved. During the discharge planning process, Anna was invaluable. She shared a few home care resources with us and made sure we had all the medical equipment we needed to help keep mom safe at home. I must have visited her office at least twice a day in the final few days. She always made time!And of course, I would be remiss to not shout-out the leadership on the nursing and clinical side. Hunter is the Nurse Manager on mom’s hall and Michelle is the Clinical Administrator for the facility. Anytime we had a concern or question they were on it. The one main complaint we had was when mom was experiencing increased confusion, all signs pointed to UTI. The first issue was that the outside lab didn’t return results for over 48 hours. I realize that’s outside Park Place’s control; however, the prelim results came in late one evening indicating UTI and the attending physician did not promptly start broad based antibiotics. I was frustrated and promptly reached out to both Hunter and Michelle. They jumped right on it and staffed with the doctor and advocated on our behalf and the antibiotic was started early next morning and UTI resolved.I appreciate, too, that Hunter and Michelle were always visible, accessible and willing to serve throughout mom’s entire stay. Also, I have tip of the hat to Michelle for being an administrator who is willing to jump in the trenches. My respect for her leadership grew when one Sunday morning she checked-in to update me on mom’s night, after she herself covered a night shift. As an administrator at a 24/7 residential Behavioral Healthcare facility, I am all too familiar with the challenges of staffing shortages and know how just one or two call outs can make things really hard. I’ve had to peel myself out of bed at 1AM to report to my facility and cover a shift. Seeing Michelle do the same let me know that Park Place has leaders who are willing to do what it takes to make sure residents in their care are well cared for. TO BE CONTINUED in Part 3.

My Family's Positive Experience, Part 1I can’t say enough about the team at Park Place. My mother transitioned here after a 23 day stay in the hospital which was precipitated by severe side effects from chemotherapy. Apart from being a Walton County resident and wanting to have mom close to home, one of the main reasons we chose Park Place is because of the high marks they receive in the Physical Therapy department. All total mom was a resident here for 44 days, and I spent a majority of my waking hours at the facility, so I can say not only do they have a top notch therapy (PT/OT/ST) team, but across the board from the administrative, admissions, nursing, direct care (CNAs), social work and transportation departments, Park Place is stacked with rock stars who truly care and are good at their craft.Our positive experience started even before mom ever transitioned to Park Place from the hospital. It started with Stephanie and Jessica in the admission department. Being a doting son who will do anything for mama, I did not sit idly by while the hospital case manager coordinated mom’s transition to a SNF. I was very proactive in calling nursing homes. Stephanie and Jessica not only responded to my calls and answered all my questions, they met with me face to face at Park Place and allowed me to tour the facility. It is worth noting, that the ambiance and aesthetics certainly stood out too; it didn’t feel or smell like a sterile environment if you know what I mean. Stephanie is extremely knowledgeable about the intake process and everything insurance. Everything worked out and mom secured a bed.As far as the care mom received, I’ll start with the Nursing Team. If ever your loved one is on E-hall for short-term rehab, I’ll bet Rapheal is their nurse. He was the main day shift nurse on E hall taking care of mom. Not only does he have good skills, he is also patient, attentive, funny and a hard-worker. Raphael constantly went above and beyond to make sure mom as well as me, my sister and wife had everything we needed even if it was something as small as a cup of crushed ice. Brittany was also great, she is a PRN nurse who’s been there 5-6+ years. She is smart, kind and has a way of putting you at ease. We had no complaints with any of the nurses but Raphael and Brittany truly stood out.On the direct care (CNA) side, there were so many team members that stood out to us. Destinee has one of the warmest smiles and most gentle demeanors you’ll encounter. Yuli was one one of the go-to CNAs for mom. Even when she wasn’t assigned mom’s room, if ever we needed anything and she was available she would jump in to assist. Makumba (I apologize if this spelling is incorrect) really stood out too. I appreciate how she would drop by and check on mom even when working on another floor. Makumba not only checked in on mom but also me and the family. Kiki is another tech who stands out in my mind. She is experienced and really seemed to be a team player, again, jumping in to help even when not assigned mom’s room. These folks are truly the unsung heroes. I think only once or twice did we ever notice mom being soiled and needing changing. If the assigned tech wasn’t available, then often I would check-in with one of the lead CNA’s, Kim, at the nurses station. I appreciate that she could always tell if I was looking for assistance and rather than finding someone else she would almost always lend a hand. TO BE CONTINUED in Part 2

Park Place Nursing and Rehabilitation Facility in Monroe is outstanding in the Nursing Home arena. The facility is spotless, shiny and clean. The residents are treated with care, love and personal attention. They are our family members. The entire team cares for our residents from delicious and nutritious meals to personally fitting activities and entertainment. We have a great group of nurses and care givers for our precious elderly population in this community. Our rehabilitation/gym in the facility is stocked with fantastic and caring therapists who are equipped with the latest machines and tools, aiding in a successful rehabilitation environment! No doubt, this is the best nursing home and rehab facility in the area. I have visited several and even volunteered at some, but this one is definitely the nicest, smoothest running, and cleanest. The staff are wonderful and super helpful. After my grandma fell and was in the hospital for weeks, we deliberated about what rehab we could trust to take the best care of her. Park Place has exceeded expectations and we are so thankful we chose it.

viewed the facility today. It is busy, but that's a great thing. (that means they must be good to have so many lovely residents) the employees are engaged in the residents. There seemed to be a nurse or cna around every corner and someone willing to help. There are many accommodations that made this a comfortable, clean, and friendly environment. I was very pleased with my visit.

My mother in law has been a resident for years , and she has received great care for her 24/7 special needs.The therapy staff even modified her wheelchair to make her more comfortable. Our family need the same care for her husband.We would love for them to be together again. The LORD willing this shall happen soon. I thank the staff for all they do !
